Link Worker – Supporting High Intensity User 

We have an exciting new opportunity for a Link Worker –  Supporting High Intensity User to join our team in Merton.  

The Merton Social Prescribing service is one of the most established and mature services in the UK and is celebrating its 6th anniversary this year. We are seeking an individual to join our successful service in Southwest London.

You will be part of a High Intensity User’s team at St Georges’ Hospital in Tooting. You will regularly attend relevant MDT meetings, but you will also make home visits and support some patients in the community. Some of your time will also be remote and you will be able to work from home too.

You will improve the health and wellbeing of patients through providing access to non-medical support that increase self-help, self-management, social engagement, and healthy behaviours, and prevent ill health. You will build and maintain effective relationships with and knowledge of local statutory and non-statutory services, local GP practices, voluntary and community organisations, and public sector partners.

About The Role | How to Apply:

· 35 hrs per week

· 12 months fixed term contract with possible extension, subject to funding

· £31,395 per annum plus 5% pension, includes Outer London Weighting

· You will receive regular Clinical Supervision
